- Proof of medical Physical-completed Health Inventory (good for one year from date taken)**
- Medical Card - Available from coaches
- Parent Permission Form
- Player Contract
- Transportation Form
- Appropriate attire/equipment depending on sport

Sports Physical Examinations will be provided at no cost to eligible Northwood High School students, by the Wellness Center Clinic. The Wellness Center Clinic is located on the first floor, B hallway. Stop by or call 301-649-8263 for an appointment.
School Based Health and Wellness Centers: School Based Health/Wellness centers provide comprehensive health, mental health and social services, case management and health promotion in familiar and student and family-friendly environments.Eligibility Requirements: Enrolled in Care for Kids, school or their siblings, aged 2 yrs. - 19 yrs. Must live in a zip code served by the school.
